A quasi-experimental study to assess the effectiveness of Risk-stratified Care Program on GI bleeding in terms of Knowledge among patients with Chronic Liver Disease

Background: Chronic liver disease is a significant global health concern, affecting approximately 292 million individuals and leading to serious complications such as gastrointestinal bleeding, particularly due to portal hypertension. There are many risk factors for bleeding in the patients with esophageal varices including elevated pressure in the portal vein, the large size of varices. Also, constipation, vomiting, severe coughing, and excessive consumption of alcohol may precipitate the rupture of esophageal varices. It is necessary to provide knowledge to the patient with esophageal varices about the disease and prevention of gastrointestinal bleeding. Objective: This study aimed to evaluate the effectiveness of a risk-stratified care program in enhancing chronic liver disease patients’ knowledge about gastrointestinal bleeding. Methods: A quasi-experimental design was employed with 150 patients, divided into an experimental group receiving the educational intervention and a control group receiving the routine care. Knowledge was assessed using a structured questionnaire before and after the intervention. Results: In pre-test, all patients in Experimental group scored below average. After the administration of program, majority of the patients (97.3%) scored good. Statistically significant increase in knowledge scores was found in the experimental group (20.71) as compared to the control group (13.20, p < 0.001). There was a significant association found between knowledge gain and variables such as educational status, alcohol consumption, and endoscopic findings of esophageal varices. Conclusion: Risk stratified care program was effective in improving patients’ knowledge about GI bleeding. Higher educational status, a history of alcohol consumption, and more severe esophageal varices demonstrated greater knowledge improvement.
